- RCGP Future vision for General Practice- it has to be evidence based. Otherwise we waste time and resources and do harm. I also want patients to be centre stage: we could be doing far more to resource shared decision making and understanding outside the consultation.
- GP Forward View report – lots of nonsense in terms of interventions that are supposed to reduce workload but there is not evidence for this – especially since the SoS has effectively told every under 40 in England to have their BP and cholesterol checked.
- Sepsis guidance – pushing for routine NEWS2 scores. I feel that we have to have have far , far better research for primary care – not research done in secondary care and presumed to work in primary care. This is dangerous – they are not reliable. When prevalence is lower you need to be very very careful that you are not being misled with poorly performing tests. This creates false positives and negatives which create harms. Sepsis is rare, minor illness is common – trying to help sort the two -especially predicting future worsening of illness – is remarkably difficult and requires high quality evidence to help us – not presumption and merely good intentions.
